Isaiah House Continuum of Care: A Phased Approach to Lasting Recovery

At Isaiah House, our treatment model has evolved into a comprehensive, tiered continuum of care designed to meet individuals where they are in their recovery journey. Formerly structured as a four-phase program, we now offer a progressive treatment path through the following levels of care:

Residential Treatment: High-Intensity

Our high-intensity residential program provides a highly structured and supportive environment for individuals who need intensive care to start their recovery journey. Clients participate in up to 20 hours of group therapy each week, along with weekly individual therapy sessions that focus on personalized goals and healing. Daily structured activities promote healthy habits, self-awareness, and personal growth. This foundational phase helps establish stability and develop essential coping skills in a safe, community-focused setting.

Residential Treatment: Low-Intensity

Building on the progress made in high-intensity care, this phase introduces more independence while maintaining structured support. Clients engage in around 10 hours of group therapy weekly, in addition to individual therapy and daily structured activities. This phase helps clients strengthen their recovery skills, develop personal responsibility, and prepare for greater engagement with daily life outside of treatment.

Partial Hospitalization Program (PHP)

Our Partial Hospitalization Program offers a strong clinical focus for individuals transitioning from residential treatment or those needing a high level of outpatient care. Clients receive 30 hours of group therapy per week, weekly individual therapy sessions, and regular treatment plan reviews to ensure personalized, effective care. This phase supports clients in beginning to navigate life with more autonomy while still maintaining intensive therapeutic support.

Intensive Outpatient Program (IOP)

Our Intensive Outpatient Program is designed for individuals ready to further integrate into their community while maintaining a structured treatment schedule. Clients participate in 12 hours of group therapy weekly (typically 3 hours per day, 4 days a week), along with weekly individual therapy and peer support sessions. Case management services and frequent treatment plan reviews help ensure continued progress. This phase empowers clients to apply new coping strategies and strengthen their recovery while balancing work, family, and other responsibilities.

Outpatient Program (OP)

Our Outpatient Program supports clients who are ready for greater independence but still benefit from ongoing connection and clinical support. Services include biweekly individual therapy sessions, weekly peer support (both individual and group), a weekly psychoeducation group, and monthly treatment plan reviews. Clients continue to receive case management and regular clinical check-ins to help sustain long-term recovery and healthy living. This final phase focuses on reinforcing lifelong skills and maintaining momentum toward a fulfilling, substance-free future.

Each level builds upon the one before it, ensuring clients receive the structure, clinical care, and peer support they need to succeed long term.
Isaiah House has proudly maintained a 75% success rate among graduates of our original four-phase model, with individuals remaining in recovery more than two years post-graduation. Our current model honors that legacy while expanding accessibility and personalizing the path to recovery. Every step of the way, our compassionate team walks beside each client, offering hope, healing, and the tools to thrive.

Education, Work & Housing

In the later stages of the recovery process, our clients have the opportunity to develop personally and professionally with our workforce development program. Clients may take GED classes, college classes, and vocational training, which are provided by our partners like Adult Education, EKCEP, Campbellsville University, BCTCS, and ECTCS. Our campuses have resources for education and job training on site.

When it comes to housing, clients are able to stay at one of our campuses designed just for men. We also have quality and clean recovery housing available. Several of these houses are certified by the National Association of Recovery Residences, and our recovery, transitional, and sober living houses are considered excellent examples for the state of Kentucky.
